Starting your practice can feel overwhelming! Writing up your own paperwork or trying to source templates is time consuming, and often it’s difficult to know what paperwork you’ll need.

Here is what the Private Practice Paperwork (PPP) includes:

  • A video explaining all of the things that need to be considered when setting up forms and policies, such as:

    • Security of the various platforms for Telehealth

    • Having a versatile COVID policy

    • The importance of a cancellation policy and how to enforce it

    • What a service agreement is and when you need them

  • Consent form - this is required prior to starting any form of assessment or counselling.

  • Telehealth Consent – there are a few different elements to consider when doing Telehealth.

  • Cancellation Policy – this is so important as it’s there needs to be a process if clients cancel.

  • COVID Policy – often clients can test positive but want to continue with their session.

  • Credit Card Storage Policy – needed to explain how their details are stored and what clients will be charged for and when.

  • Consent to Exchange Information – required if you need to speak to other health care professionals.

  • Family Consent Form – what clients can expect when you are conducting family therapy .

  • Couples Consent Form – explains how therapy will work and which clients are suitable for couples therapy.

  • Service Agreement Template – this can be converted to be used for NDIS, Open Arms etc. These involve a combination of the above and are specific to the provider. This document is generic and can be adapted for use specific to the provider involved.
